JOB SUMMARY: Responsible for conducting telephonic or face-to-face assessments for the identification, evaluation, coordination and management of Members’ needs, including physical health, behavioral health, social services and long term services and supports; develops the Member’s Individualized Service Plan to address those needs. Establishes relationships with referral sources and community resources, while maintaining strict member confidentiality and complying with all HIPAA requirements.

Additional Federal Attention Needed to Help Protect Access for Students with Disabilities

Charter schools enrolled a lower percentage of students with disabilities than traditional public schools, but little is known about the factors contributing to these differences. In school year 2009-2010, which was the most recent data available at the time of our review, approximately 11 percent of students enrolled in traditional public schools were students with disabilities compared to about 8 percent of students enrolled in charter schools.

HUD Makes Available $85 Million To Fund Housing For Extremely Low-Income Persons With Disabilities
New Section 811 demonstration links state housing agencies with state Medicaid agencies

Tuesday, May 15, 2012

WASHINGTON – Today, the U.S. Department of Housing and Urban Development makes $85 million available to state housing agencies to provide affordable supportive housing for extremely low-income persons with disabilities. This is the first time in the history of HUD’s Section 811 Supportive Housing for Persons with Disabilities Program that funding will be offered to state housing agencies that meet new eligibility criteria, including having a partnership with a state health and human services and Medicaid agency to provide essential support and services. The Notice of Funding for the Section 811 Project Rental Assistance was posted on Grants.gov today here.
